Fairview Health Services is seeking an experienced Psychiatric Mental Health Nurse Practitioner (PHMNP) with experience and strong interest in providing outpatient psychiatry care to adolescent patients at the Fairview Dual Diagnosis Adolescent Day Treatment Program in Crystal, MN.
In this collaborative and interdisciplinary practice model, the PMHNP will be working with a team comprised of program staff, teaching staff, chemical dependency counselors, and psychotherapists. The PMHNP will be actively involved in all patient care including intakes, follow-ups, medication management, and conducting family conferences. It is a very team-oriented approach, where the PMHNP is the leader of the treatment team, with support in discussing what clinically is most appropriate for each patient.
The PMHNP will be part of a day treatment provider group that consists of psychiatrists and other nurse practitioners. While the majority of this position is spent at our Crystal location, a portion of time may be for helping see patients in our Adolescent Dual Diagnosis residential program, as well as occasionally (few times per year) helping cover at our other day treatment sites.
This program is affiliated with the University of Minnesota Masonic Children's Hospital, home to one of the largest inpatient mental-health and substance abuse programs for children and teens in the nation. Our wide range of services includes: inpatient, behavioral emergency center, partial hospitalization, outpatient clinics, counseling centers, adolescent chemical dependency program, and adolescent dual diagnosis day programs.
Graduate of a school or nursing and accredited Adult or Family (Lifespan) Psychiatric & Mental Health Nurse Practitioner or Clinical Nurse Specialist training program
BC/BE through the AANP or ANCC
Licensed or ability to obtain APRN license with the Minnesota Board of Nursing
Ability to obtain and /or maintain DEA certification for the State of Minnesota.
