  • Practice Description

    Fairview Health Services is seeking a Pain and Palliative Care Clinical Nurse Specialist Supervisor to provide lead advanced practice providers and provide clinical care at Fairview Ridges Hospital.  As the supervisor, lead a care team of 3 Nurse Practitioners/Clinical Nurse Specialists who round on an average daily census of 12-20 patients; share the responsibility of new consults as well as inpatient follow-up care in hospital setting and the potential for outpatient clinic follow-up. Provide consult service for both pain and palliative care as part of an interdisciplinary team consisting of a chaplain, social worker and physicians, to include a close working relationship with the Hospitalist group.

     

     

    Practice Details:

    • Fairview Ridges Hospital is a 150 bed Level III Trauma facility.
    • Pain and Palliative care program established in 2005.
    • Patient encounters average 12-20; 5-7 consults per day.
    • Support of decentralized pharmacy with Pharm D available for consult. 
    • Call Frequency: Currently no call obligation.
    • Enjoy a Monday through Friday work schedule.
    • Openings: 0.8 FTE
    • As a Pioneer Accountable Care Organization (ACO), our physicians use a multidisciplinary team approach in managing clinical care.
    • Fully integrated Electronic Medical Record (EPIC).

    Qualifications

    Minimum Qualifications: 
    • Graduate of a school of nursing and accredited Family, Adult/Geriatric, or Acute Care Nurse Practitioner training program
    • BC/BE through the AANP or ANCC
    • Licensed or ability to obtain APRN license with the Minnesota Board of Nursing
    • Ability to obtain and /or maintain DEA certification for the State of Minnesota.
    • Meet threshold criteria/qualifications for Credentialing and Privileges.

     

     

    Preferred Qualifications:

    Minimum of 5 years previous, relative pain and palliative care experience in areas that could include geriatrics, med/surg, oncology, critical care, family medicine, etc. Certification in either Pain or Palliative Care, with the goal and expectation to become certified in both areas. 1-3 years of supervisory experience.
