  • Practice Description

    M Health Fairview Clinic -  Grand Avenue is seeking a Family Medicine physician (OB is optional) to join our primary care practice.  Thi clinic is nestled in the Summit Hill neighborhood of St. Paul, and serves a variety of patients from pediatrics to geriatrics, including professors, college students, immigrants and refugees.  



    • Primary care clinic with 6 Family Medicine physicians and a Nurse Practitioner
    • Other services  offered in the clinic include Psychotherapy, OB/Gyn, CNM, MTM pharmacist, diabetes education and care coordination.
    • Full scope family medicine practice in an outpatient clinic setting
    • Opportunity to provide care for a diverse panel of patients.
    • Phone Call Only is 1:11 (without OB). 
    • If interested in OB, physicians deliver their own babies, and are on back up OB call. 
    • Patient encounters average between 18 – 22 patients per day.
    • Full time (1.0 FTE), 32 patient contact hours per week
    • Clinic hours are 7:30am-5pm, Monday - Friday.
    • A fully integrated electronic medical record (Epic) is utilized.

    • Graduate of an accredited ACGME/AOA Family Medicine residency training program.
    • BC/BE with the ABMS or AOA.
    • Licensed or ability to obtain licensure through the Minnesota Board of Medical Practice.
    • Ability to obtain and /or maintain DEA certification for the State of Minnesota.
    • Meet threshold criteria/qualifications for Credentialing and Privileges (if required).
