Practice Description

Fairview Health Services is seeking a Hospice Physician to help provide medical direction for our large, established Hospice program. Hospice treats the physical and emotional symptoms experienced at the end of life. The Hospice multidisciplinary team includes physicians, nurse practitioners, nurse clinicians, clergy, social workers, music therapy and massage therapy.

 

Practice Details:

  • Seeking a provider interested in helping to create new models of hospice care.
  • Schedule: Days – Monday through Friday
  • New operational strategic partnership with a leading national home care/hospice company (AccentCare).
  • Cover the entire system network – great majority of hospice care is delivered in the home or senior care facility.
  • Call Frequency: Call has variable intensity, shared for evenings/nights/weekends.
  • Openings: 1.0 FTE

Qualifications

Qualifications:

  • Graduate of an accredited ACGME/AOA Internal Medicine, Family Medicine, IM/Peds, or Emergency Medicine residency training program.
  • BC/BE with the ABMS
  • Licensed or ability to obtain Minnesota Board of Medical Practice Licensure.
  • Ability to obtain and /or maintain DEA certification for the State of Minnesota.
  • Meet threshold criteria/qualifications for Credentialing and Privileges.

 

Preferred Qualifications:

  • Board certification in Hospice/Palliative Care preferred, but not required.  We are willing to provide training to applicants without prior hospice experience who have a passion for end of life care, are committed to lifelong learning, and enjoy working in a multidisciplinary team.
